Anyone interested in exploring naturopathic medicine for the first
time is welcome to schedule a complimentary 15 minute consultation to discuss whether naturopathic medicine is right for them.

Once you have scheduled your first appointment, please bring the following paperwork and information with you:

• Recent blood-work results or medical reports (if applicable)
  
  
• A completed Adult Intake form or Pediatric Intake form
  (if the patient is under 14 years of age)
  
  
• A list of medications, supplements, and vitamins
  you are taking or have taken in the past.
  
  
• Contact information for other healthcare providers
  you are consulting.

You may also obtain these forms from the front reception desk. Please note that the intake form requires an average of 25 minutes to complete.

First appointments may last 60-90 minutes and may involve
a physical exam and laboratory testing. Follow-up appointments generally last between 15 and 30 minutes and will be recommended according to your particular health requirements.

Consultation fees do not include the cost of prescription items or laboratory tests, as these are recommended on an individual basis.
Fees do not include HST. Payment may be made by Visa, Mastercard,
or debit. Please provide at least 24 hours cancellation notice to avoid
a $30 service fee.
